Typhoid Fever

Typhoid fever is a serious and potentially life-threatening illness caused by the bacterium Salmonella Typhi. It spreads through contaminated food or water and is especially common in regions with poor sanitation. Though rare in countries like the U.S., it affects millions globally each year.

What Is Typhoid Fever?

Typhoid fever is an acute bacterial infection marked by prolonged high fever and various systemic symptoms. It is primarily caused by Salmonella enterica serotype Typhi and, less commonly, Salmonella Paratyphi.

The bacteria are spread through contaminated food and water, often in areas with inadequate sanitation. Once ingested, they multiply in the intestines and enter the bloodstream, leading to infection throughout the body.

How Common Is It?

In the early 1900s, typhoid was widespread in the U.S., but improved sanitation drastically reduced cases. Now, fewer than 400 cases are reported yearly in the U.S., primarily among travelers returning from high-risk areas such as South Asia, Africa, and parts of South America.

Globally, typhoid fever affects more than 21 million people each year and causes an estimated 200,000 deaths.

Signs and Symptoms

Typhoid fever symptoms usually develop gradually—starting 6 to 30 days after exposure. Common symptoms include:

• Headache
• Fatigue and weakness
• Muscle aches
• Dry cough
• Loss of appetite and weight loss
• Abdominal pain
• Diarrhea or constipation
• Rash
• Fever up to 104°F (40°C)
• Swollen stomach

In many cases, chest congestion and severe abdominal discomfort occur. If untreated, complications can appear during the third or fourth week. About 10% of patients may experience a relapse within 1–2 weeks of recovery, especially those treated with antibiotics.

Causes and Risk Factors

Typhoid fever is caused by the bacterium Salmonella Typhi, transmitted primarily via the oral-fecal route. It spreads when food or drinking water becomes contaminated by an infected person’s feces or urine.

Some individuals become asymptomatic carriers after recovery and can unknowingly transmit the disease.

Risk is highest in areas with inadequate access to clean water and sanitation—especially in parts of Africa, India, Southeast Asia, and South America.

Complications and Risks

Severe cases of typhoid can lead to life-threatening complications such as intestinal bleeding, perforation, or infections in other organs. Antibiotic resistance is also on the rise, making treatment more difficult in some regions.

Without prompt treatment, typhoid can be fatal. Early diagnosis and proper antibiotic use are essential to prevent complications.

Diagnosis and Treatment

Typhoid fever is diagnosed through blood or stool tests. Once confirmed, treatment typically involves antibiotics. However, growing resistance to certain antibiotics is a growing concern worldwide.

Supportive care such as hydration, rest, and nutrition also plays a key role in recovery.

Prevention

Good hygiene and safe food practices are key to preventing typhoid fever. Boil drinking water, wash hands regularly, and avoid street food when traveling in high-risk regions.

Two main types of vaccines are available: oral and injectable. A newer typhoid conjugate vaccine (TCV) offers longer-lasting protection and has been endorsed by the WHO.

Who’s Most at Risk?

Typhoid fever mostly affects people in developing countries with limited access to clean water and sanitation systems. Children, the elderly, and people with weakened immune systems are particularly vulnerable.

Urban overcrowding, climate change, and inadequate infrastructure are driving a resurgence in some regions.

Global Burden of Disease

According to the World Health Organization (WHO), typhoid fever affects between 11 and 20 million people annually and causes up to 161,000 deaths per year.

While it’s under control in industrialized nations, it remains a serious public health threat in many parts of the world.
